CALGARY — Alberta Health says it has sent a letter of apology to a Calgary woman who was told that her health records indicated she was dead.
Tamille Holloway was stunned when she went to her doctor last week and staff showed her the notification from Alberta Health saying she had died.
The 28-year-old woman thought the mix-up might have happened because her identification was stolen a few years ago.
On Thursday, Alberta Health issued a statement saying it was to blame for the error.
It says when Holloway contacted Alberta Health on Dec. 2 to order a replacement health card, the agent “made a data entry error” in the system.
Alberta Health says it has put safeguards in place to prevent such mistakes from happening again.
